Actualización Guía de Práctica Clínica PANCCO para el tratamiento de la colitis ulcerativa en adultos

Resumen

Actualizar las recomendaciones de la Organización Panamericana de Crohn y Colitis (PANCCO) para el tratamiento de la colitis ulcerativa (CU) moderada a grave en adultos, incorporando evidencia reciente sobre nuevos biológicos y moléculas pequeñas. Se aplicaron los métodos de desarrollo rápido de guías GRADE. PANCCO adaptó la Guía colombiana de Gastroenterología 2025 para upadacitinib, tofacitinib, ozanimod y cambio a formulaciones subcutáneas, y elaboró de novo las recomendaciones para etrasimod, guselkumab, risankizumab, ustekinumab, mirikizumab, filgotinib y apendicectomía. La búsqueda se ejecutó en MEDLINE, Embase, Cochrane y Epistemonikos hasta febrero de 2026, evaluando la calidad con ROBIS, AMSTAR-2, RoB 2.0 e ICEMAN; la fuerza de las recomendaciones se estableció con GRADE. Se formularon recomendaciones para 9 preguntas. En CU moderada a grave se recomiendan upadacitinib, tofacitinib, filgotinib, ustekinumab, mirikizumab, risankizumab y guselkumab; se sugieren ozanimod y etrasimod. En pacientes con respuesta a inducción intravenosa, se sugiere cambio a vedolizumab subcutáneo o infliximab subcutáneo. En CU refractaria a terapias avanzadas se sugiere considerar la apendicectomía laparoscópica como estrategia adyuvante. Esta tercera actualización amplía el armamentario terapéutico para CU moderada a grave en Latinoamérica. La elección debe individualizarse según terapia previa, comorbilidades, riesgo cardiovascular y trombótico, disponibilidad local y preferencias del paciente.
